In this role, you’ll manage a client caseload, coordinate housing placements, provide direct support in the community, and help clients pursue jobs, volunteer work, or retraining opportunities. - Bachelor’s degree in Sociology or related field OR equivalent experience
- At least 2 years Independent Living Services, or 4 years individuals with disabilities
- Strong attention to detail with a proven ability to meet deadlines
- Clear and effective communication skills (written and verbal)
- Proficiency with Excel, Microsoft Office, Windows, and PDF applications
- Must have a personal computer, quiet workspace, and reliable transportation
- Ability to maintain confidentiality and handle sensitive information appropriately
- Clean driving record and ability to travel across counties
- Comfortable with approximately 25% community-based work
